Non-Compliance Equipment Reporting

The BDIA collects reports of counterfeit or non-compliant products to support its engagement with the MHRA by providing a better picture of the extent of the issue of non-compliance in the dental sector and the types of products that are affected. We encourage members to use the form below to report instances of non-compliance, including where these have already been reported directly to the MHRA. The BDIA will not normally file a further individual report if the product has already been reported by a member, but central collation of this information helps us to understand and communicate the issue to the regulator

Please note that all submissions will be treated in confidence. Contact details are requested for internal BDIA use only and will not be shared externally.

Product NameName of the product - This should include brand and model

ManufacturerManufacturer Name - indicating the legal manufacturer wherever possible.

Product DescriptionShort description of the product i.e. high speed handpiece, glass ionomer, 3D printing resin etc.0 / 500

Whether product has CE, UKCA or other certification

Whether the manufacturer is registered with the MHRA.

Registration number of the manufacturer on the PARD database (if applicable).

Until full functionality of PARD is introduced, individual product registration cannot be directly confirmed. However, if the product being reported is not covered by the GMDN codes listed under the manufacturer's registration it can be inferred that the product has not been registered.

Retailer(s)Names of retailers where the product is listed for sale. Includes the manufacturer in the case of direct sales.

Product URLURL of the product for sale. If there are multiple SKUs of the same/similar product, please enter one example and highlight this in the "notes" field.

Details of non-complianceShort description of the type of non-compliance - e.g. lack of MHRA registration, lack of certification, counterfeiting, parallel/grey importation.

Member report date DD/MM/YYYDate product was reported by the BDIA member (if applicable).

Whether the product has been reported to the MHRA by the BDIA. BDIA will not normally file a further individual report if the product has already been reported by a member.

MHRA confirmed receiptDate receipt of the member/BDIA report confirmed by the MHRA (either an automatic or human confirmation).

Whether any action has been taken against the reported non-compliance - either as advised by the MHRA or observed by others (e.g. product can be seen to have been withdrawn from sale).

Date product was last availableDate product was last seen to be available for sale. To be kept updated by BDIA, with support of ongoing input from BDIA members.

Additional notes or commentsAny further information about the product, retailers or details of the non-compliance that may be useful.
